Mental Health After Long Years on Alert

He is home. Reserve duty has ended, the sirens have almost stopped and life is supposed to return to normal. Everyone says he looks well. During the day, he does function: work, children, shopping. But at night, he cannot sleep. A week, two weeks, a month. Every loud noise outside still makes him jump from his chair. He tells nobody, because "others went through worse."
Years of sirens, reserve duty, distressing news and constant worry do not end with a single day of relative quiet. That is one of the most confusing things about mental health: difficulties often arrive later. The body is already home, but sleep, patience and calm have not returned. Israel's public system was not built in advance for this volume of requests. People seeking help therefore need more than general agreement that "it is legitimate." They need a route to care.
"Others went through more"
Let us pause on that sentence, because it is one of the most common barriers to seeking help. Comparing yourself with others sounds modest and responsible. In practice, it delays treatment. Your difficulty does not have to be "the greatest" to deserve care. It only has to interfere with your life.
Signs that justify seeking help
How do you know it is time? Not by diagnosing yourself online, but by asking a simple question: has something changed and failed to return to normal? Sleep disrupted for weeks. Avoiding work, friends or even your children. Irritability and impatience that were not there before. Feeling disconnected, or thinking there is no point.
Any of these is a good reason to make an appointment with your family doctor or contact a support service. None is a diagnosis. Diagnosis is a professional's work, after an actual conversation. Your task is simply to get to that conversation.
The first step is not always a psychologist
Here is a detail that surprises many people: your first contact can be your family doctor. They can refer you to the appropriate professional and rule out a physical problem that may appear to be anxiety. Thyroid problems, for example, or medication side effects can resemble psychological difficulties.
Health funds provide mental health services, with appointments and eligibility defined by each fund. Nonprofits and helplines, such as ERAN at 1201, can help people through a difficult night. They do not replace ongoing treatment, but they can make the difference between spending the night alone and having someone on the other end of the line.
What about reservists, bereaved families and people affected by security incidents? Dedicated routes are often available through various organizations. Do not assume there are none. Ask your health fund and your unit. The answer may surprise you.
Private, group or brief treatment: Choose without getting stuck
One thing that delays treatment is the search for "the right treatment." The perfect therapist, the perfect method, the perfect price. Meanwhile, months pass.
Private treatment is more readily available and more expensive. Check whether supplementary insurance contributes to the cost. Group therapy costs less and suits some people, especially those who feel nobody understands them. It is less suitable for someone who needs complete quiet and privacy. Short courses of sessions also focus on particular problems, such as sleep or anxiety.
The most important rule: a short course starting now is better than endless waiting for ideal treatment. You can change therapists if you do not connect. You cannot recover months in which you never started.
What to say at home
What if it is someone you love rather than you? No lecture or diagnosis is needed. A simple, sincere question, such as "How have you been sleeping lately?" often opens more doors than a speech about the importance of treatment. Make room for the answer. Do not rush to fix things.
If there is a real concern about self‑harm, do not wait for an appointment a month away. Go to an emergency department, contact an emergency service or call Magen David Adom at 101. This is not a discussion about character or weakness. It is an urgent medical situation, just like chest pain. If you feel this way yourself, you do not have to go through it alone. Even now, tonight, someone will answer.
